Rates and Policies

2021 Regular Rates:

30 min. $60 / 60 min. $100 / 90 min. $140 / 120 min. $180

Rates are “all inclusive,” there are no additional membership fees; no upcharge for deep tissue, advanced techniques, evening/weekend hours, or product used (aromatherapy, CBD, etc.); and no additional gratuity is accepted.

 

~Face Covering: A mask or face covering is required at all times as per DORA, unless medically exempt. I am comfortable working with exempt individuals, and anyone not using a face covering I will assume is claiming exemption.

 

~Illness: A fever is always contraindicated with massage, even if the temperature is artificially brought down with medication (tylenol, Advil, etc.). Cancellations in the event of any illness are always encouraged. If you answer “YES” to any of these questions, your appointment must be postponed:

 

  • Have you had a fever in the last 24 hours of 100°F or above?
  • Do you now, or have you recently had, any respiratory or flu symptoms, dry cough, sore throat, or shortness of breath?
  • Have you been in contact with anyone in the last 14 days who has been diagnosed with COVID-19 or has coronavirus-type symptoms?

 

~Cancellations: 2 hours advance notice is required, and 24 hours advance notice is preferred for all cancellations, with the exception of emergencies, illness, serious injury, or inclement weather (at your discretion). If you are unable to provide the required 2 hours advance notice, you will be charged 50% of the amount of your appointment, and this amount must be paid prior to your next scheduled appointment. 

~No-shows: Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a "no-show". They will be charged for their missed appointment and future service will be postponed until payment is made. 

~Punctuality: Arriving in the 10 minute window of ~5 minutes before to ~5 minutes after your scheduled appointment time will allow for the full length of your treatment while allowing time for proper sanitation before you arrive. Appointment times have been arranged specifically for you. If you arrive late your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the "full" session.



 

Consent for Treatment 

If I experience any pain or discomfort during this session, I will immediately inform the practitioner so that the pressure and/or strokes may be adjusted to my level of comfort. I further understand that massage/bodywork should not be construed as a substitute for medical examination, diagnosis, or treatment and that I should see a physician, chiropractor, or other qualified medical specialist for any mental or physical ailment of which I am aware. I understand that massage/bodywork practitioners are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness, and that nothing said in the course of the session given should be construed as such. Because massage/bodywork should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions and answered all questions honestly. I agree to keep the practitioner updated as to any changes in my medical profile and understand that there shall be no liability on the practitioner’s part should I fail to do so. I also understand that any illicit or sexually suggestive remarks or advances made by me will result in immediate termination of the session, and I will be liable for payment of the scheduled appointment. I understand that, because massage therapy work involves maintained touch and close physical proximity over an extended period of time, there may be an elevated risk of disease transmission, including COVID-19.  I understand that I am the decision maker for my health care. To the best of their ability, my practitioner will provide me with information to assist me in making informed choices. This process is often referred to as “informed consent” and involves my understanding and agreement regarding recommended care, and the benefits and risks associated with the provision of health care during a pandemic. Given the current limitations of COVID-19 virus testing, I understand determining who is infected with COVID-19 is exceptionally difficult. I understand that preventative measures and intensified sanitation protocols intended to reduce the spread of COVID-19 have been implemented. However, because this work involves close physical proximity over an extended period of time in a closed space, there may be an elevated risk of disease transmission, including COVID-19. I hereby acknowledge and assume the risk of becoming infected with COVID-19 through this treatment and give my express permission to you and the staff at your offices to proceed with providing care.

 

By using our services, I acknowledge that I am aware of the risks involved and give consent to receive massage and bodywork from this practitioner. Understanding all of this, I give my consent to receive care.
